In her native form Azalea is a curious blend of enticing and monstrous, the latter enhancing or diminishing the former depending on one's preferences. She stands at just a little under six feet tall and posseses a pleasantly soft build overall. Her facial features are similarly soft and strikingly pretty, with the elven ears adding to the already otherworldly look. Her many fiendish features quickly shatter any notion of her simply being a tall elf with skin tinged a pale blue. A single, lidless red eye rests on her forehead, the color matching those of her normal eyes. Her arms and legs are curious, the hands and feet being blackened and withered. Streaks of black travel up her arms and legs, stopping entirely at the elbows and knees. Horns, wings, and a scaled tail ending in a serpent's head round out the fiendish look.

Her arms and legs are decorated with a mixture of scars and tattoos, most of them depicting intricately winding thorned vines. She's rarely seen without two pieces of jewelry, the first an expensive and ornate wedding ring with fey themes etched on its band. The second is a golden heart shaped pendant with an exquisitely carved fairy made from a glossy black gem at its center, flecked with bits of red. There's a macabre touch to it in that if one looks closely the necklace isn't worn around her neck.  The chain is embedded into her skin at the back of her neck.
